29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:    (a) Building 1, laminator area; The energy isolation procedures were not evaluated at least annually for multiple energy source machines including, but not limited to; Almex and Italpresse brand laminator presses.    The Stratis Industries dba Sunway, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 1227050, citation number 3, item number 1 and was affirmed as a final order on May 16, 2017, with respect to a workplace located at 321 E. 1st Street, Centuria, WI 54824.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee did not affix a personal lockout or tagout device to the group lockout device before working on the machine or equipment:  (a) Building 1, laminator area; Each person did not affix their own lockout lock on the electrical and pneumatic energy isolation points during membrane changing operations on the Almex brand, laminator press.
