29 CFR 1910.23(b)(8): Ladders was not used for the purposes for which they were designed: Crane Assembly: On or about June 24, 2019 and at times prior thereto, a six-foot step ladder used to access the operations cab of the rubber-tired P&H crane was used as a straight ladder. This condition exposed the employee of a fall hazard of up to five feet.

29 CFR 1910.184(c)(6): Slings were not securely attached to their loads: Liebherr HS 8130 Assembly Area: On or about June 24 2019 and at times prior thereto, during the lift and movement of the crawler-weight, wire-rope sling hooks were not securely attached to the load. Hooks were turned in. This condition exposed employees to struck-by hazards.
