29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.  On or about June 28, 2019, an employee was observed using a step ladder on top of a frame scaffold platform to increase the reach while performing paint primer work.  This condition exposed employees to fall hazards greater than 10 feet from the ground.
