29 CFR 1910.333(b)(2)(i): The employer did not maintain a written copy of the procedures outlined in paragraph (b)(2) and did not make it available for inspection by employees and by the Assistant Secretary of Labor and his or her authorized representatives. This violation occurred on or about June 24, 2019, the employer did not ensure that specific written lockout/tagout procedures were developed for the control of potentially hazardous energy when performing servicing on the MCC cabinet, exposing employees to the hazards of arc flash.

29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. This violation occurred on or about June 24, 2019, the employer did not ensure that the arc flash protective clothing rated at 40 Cal were laundered per the manufacturers requirements, exposing the employees to the hazards of arc flash.
