29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Jobsite, 107 High Pointe Drive, Washington, PA: On or about July 1, 2019, two employees who were working in an excavation (approximately 40 feet in length) that ranged in depth from 5 1/2 feet to 11 1/2 feet did not have a ramp, ladder or other safe way to exit the excavation. (b) Jobsite, 151 Piatt Estates Drive, Washington, PA: On or about July 1, 2019, an employee was working in an excavation (approximately 40 feet by 50 feet) that ranged in depth from 5 feet 2 inches to 6 feet did not have a ramp, ladder or other safe means to exit the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) Jobsite, 107 High Pointe Drive, Washington, PA: On or about July 1, 2019, two employees were working in an excavation that ranged in depth from 5 1/2 feet to 11 1/2 feet in depth and there was no shoring nor sloping to prevent a cave-in. (b) Jobsite, 151 Piatt Estates Drive, Washington, PA: On or about July 1, 2019, an employee was working in an excavation that has sidewalls that measured between 5 feet 2 inches to 6 feet in depth. This excavation was about 40 feet by 50 feet. The employee was exposed to a cave-in when the employee would enter and exit the excavation.
