1926.652(b)(2) Option (2) - Determination of slopes and configurations using Appendices A and B. Maximum allowable slopes, and allowable configurations for sloping and benching systems, shall be determined in accordance with the conditions and requirements set forth in appendices A and B to this subpart. 29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). a) Jobsite- Route 19 & Old Route 19, Cranberry Township, PA: On or about January 17, 2020, employees working in an excavation that measured approximately 7 feet in depth where the employee was located and additional 9 feet in depth to the bottom of the excavation, were not provided with adequate cave-in protection. The Demex Demolition & Excavation, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1412454, citation number 1, item number 2 and was affirmed as a final order on 10/01/2019, with respect to a workplace located at 107 High Pointe Drive, Washington, PA 15301.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) Jobsite- Route 19 & Old Route 19, Cranberry Township, PA: On or about January 17, 2020, employees working in an excavation that measured approximately 7 feet where the employee was located and additional 9 feet to the bottom of the excavation, were not provided a safe means of egress out of the excavation. The Demex Demolition & Excavation, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 1412454, citation number 1, item number 1 and was affirmed as a final order on 10/01/2019, with respect to a workplace located at 107 High Pointe Drive, Washington, PA 15301.
