29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about June 25, 2019, during the maintenance (pressure washing) of a school roof located at 111 Stadium Dr, Youngstown, Ohio 44512, an employee fell approximately 8.6 feet inches to the ground below due to the employer not ensuring that the employees on a roof performing work activities were protected from a fall by implementing the use of personal protective equipment.
