29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:  On or about October 28, 2020, employees were exposed to a fall of approximately 12.5 feet while performing roofing activities on a commercial building located at 9655 Valley View, Valley View, Ohio 44125.  The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection.    Tema Roofing Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910. 132 (a), which was contained in OSHA inspection number 1412675, citation number 1, item number 1 and was affirmed as a final order on 9/5/2019, with respect to a workplace located at 111 Stadium Drive, Youngstown, Ohio 44512.
