29 CFR 1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part:  Employees were exposed to struck-by and fall hazards.  At the jobsite located 1729 E. 84th St. S. Haysville, Kansas:  Employees engaged in roofing activities were exposed to struck-by and fall hazards when working without eye protection while using pneumatic nailers, accessing roof with improper ladder extension, and working from heights greater than 6 feet above a lower level.  Employees were exposed in that the employer had not developed and maintained a written comprehensive safety and health program which addressed these and other hazards.

At the jobsite located at 1729 E 84th St S, Haysville, Kansas:  Employees engaged in residential roofing activities, walking and working on a 6:12 pitch roof, were exposed to fall hazards of approximately 10 feet 6 inches from the eave of the residence to the ground below in that fall protection was not utilized.

At the jobsite located at 1729 E. 84th St. S. Haysville, Kansas:  Employees engaged in residential roofing activities from a 6:12 pitch roof were exposed to fall hazards of approximately 10 feet 6 inches from the eave of the residence to the ground below in that the ladder used to access the roof was not extended at least 3 feet above the eave.
