29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a) 752 Frank St., Emmaus, PA, Worksite, Roof - On or about June 28, 2019, employees were performing residential roofing work on a 7-12 pitch roof, between 10 to 23 feet above grade, without utilizing any means of fall protection.        Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.  Jaw Contracting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1327834, citation number 1, item number 1 and was affirmed as a final order on September 10, 2018, with respect to a workplace located at 1421 W. Union Blvd., Bethlehem, PA.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) 752 Frank St., Emmaus, PA, Worksite, front of house - On or about June 28, 2019, employees were using an extension ladder that did not extend at least three feet above the upper landing surface to which the ladder was used to gain access to the front of the house, for purposes of performing residential roofing work.   b) 752 Frank St., Emmaus, PA, Worksite, backside of house - On or about June 28, 2019, employees were using an extension ladder that did not extend at least three feet above the upper landing surface to which the ladder was used to gain access to the backside of the house, for purposes of performing residential roofing work.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. 29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a) 752 Frank St., Emmaus, PA, Worksite, backside of house - On or about June 28, 2019, employees performing residential roofing work were using an extension ladder with a highly visible structural defect that had not been removed from service for disposal or repair.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
