29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):    (a)Site Work Specialists, Inc. at Disk Drive and Haines Avenue Project in Rapid City, SD 57701: On or about and at times prior to June 28, 2019, the exposing employer did not ensure an employee performing work in an excavation at depths greater than five feet was protected from cave-in hazards. The employer did not ensure that an appropriate protective system was installed or used near the southeast face of the excavation used to access and egress the excavation by ladder. The benching installed in this area was not designed in accordance with 29 CFR 1926.652(b). This condition exposed one employee to cave-in hazards.    (b)Site Work Specialists, Inc. at Disk Drive and Haines Avenue Project in Rapid City, SD 57701: On or about and at times prior to June 28, 2019, the exposing employer did not ensure a employees performing work in an excavation at depths greater than five feet were protected from cave-in hazards. The employer did not ensure that an appropriate protective system was installed or used near the east face of the excavation where three employees performed work in an unprotected excavation to tie-in underground utilities. The benching installed in this area was not designed in accordance with 29 CFR 1926.652(b).This condition exposed three employees to cave-in hazards.
