29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 1115 Wolcott Ave. - On or about July 1, 2019 and at times prior thereto, employees were exposed to approximately 20' fall hazards while framing the 3rd story walls and floor.  No personal fall protection was being used and there were no guard rails installed.   b) 1115 Wolcott Ave. - On or about July 1, 2019 and at times prior thereto, employees were exposed to approximately 10' fall hazards while using the 2nd floor as a staging platform for materials and as an access point to the rest of the building.  No personal fall protection was being used and there were no guard rails installed.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:   a) 1115 Wolcott Ave. - On or about July 1, 2019 and at times prior thereto, employees were exposed to multiple fall hazards throughout work site.  The employees had received training in fall hazards but the employer could not verify such training with a written certification.
