29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a. On or about February 12, 2019, the employee operating a nail gun to sheath a 6:12 pitch roof was not wearing eye protection, exposing himself to flying debris.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:  a. On or about February 12, 2019, an employee of BDM Construction was sheathing a 6:12 pitch roof without being protected from 22 feet 8-inches to the floor by the use of a fall protection system.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:  a. On or about February 12, 2019, at the rear elevation of the home, a wooden job-made ladder used to gain access to the roof did not have side rails that extended 3 feet above the roof eve and exposed an employee to a 9-foot fall hazard.

On or about February 12, 2019, at the rear elevation of the home, an employee descended from the roof using a wooden job-made ladder which he was not facing and was exposed to a 9-foot fall hazard.
