29 CFR 1926.20(b)(2):  Such programs did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer.  D & D Roofing did not conduct an adequate inspection of job site, materials and equipment. During the inspection, employees were exposed to the following hazards:  a) Three employees were exposed to fall hazards without proper fall protection.  b) One employee was operating a nail gun without eye protection.   c) Three employees used a ladder to access the roof which was not extended three feet beyond the landing or edge of the roof.

29 CFR 1926.102(a)(2): The employer did not ensure that each affected employee used eye protection that provided side protection when there was a hazard from flying objects:  Workers at 951 Annabrook Park Drive, O'Fallon, Missouri, an employee of D & D Roofing was using a nail gun without wearing eye protection.

29 CFR 1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At 951 Annabrook Park Drive, O'Fallon, Missouri, workers from D & D Roofing were working on the roof of a residential building. The owner and two other employees were working on a roof with a ground to eave height of approximately 9 feet and a 6:12 slope. This violation was noted on June 24, 2019.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access.  Employees of D & D Roofing used an improperly extended ladder to access the roof at 951 Annabrook Park Drive, O'Fallon, Missouri.  The ladder was not extended 3 feet beyond the roof's landing.
