(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: a) Guardrail systems; b) Safety net systems; c) or Personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems. a) On or about July 8, 2019 operators on the Ferry Roto Mold Machine 280 were exposed to falls of approximately 5' 6" when removing parts from the molds while standing on the arm platform. b) On or about July 8, 2019 operators on the Ferry Roto Mold Machine 330 were exposed to falls of approximately 6' when removing parts from the molds while standing on the arm platform.

(1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart. The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following: Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) On or about July 8, 2019, the employer did not ensure that machine operators were trained to isolate energy sources and render equipment inoperative prior to allowing them to perform maintenance and set up activities on the three Ferry Mold Machines.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a) On or about July 8, 2019 on the Ferry 280 and Ferry 330 mold machines, employees, such as operators performing set up activities, did not lockout the electrical and pneumatic disconnects when making mold changes. Employees were not required to: (1) turn off and shut down the machines, (2) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the electric and pneumatic energy sources, (3) affix lockout or tagout devices to each energy isolating device, (4) verify effectiveness of energy isolation.

29 CFR 1910.212(a)(1): "Machine guarding" Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a) On or about July 8, 2019 the four Motionmaster CNC machines did not utilize a guarding method that would prevent employees from getting caught in the rotating tooling and/or stuck by the moving table. b) On or about July 8, 2019 the Rotoline machine did not utilize a guarding method that would prevent employees from getting struck by the two rotating arms on the machine.
