29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: a) Crown Well Services at 500 County Road 7 in Erie, CO 80516: On or before July 2, 2019 the employer did not ensure that employees were protected from fall hazards while working on the rig floor of Crown Well Services Rig 1. The guard rail or rail chain that protects employees from falling down the V door had been taken out of service but not replaced. This condition exposed employees to fall hazards of approximately 5 feet 3 inches.

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. (a) Crown Well Services LLC at 500 County Road 7 in Erie, CO 80516: On or before July 2, 2019, the employer did not provide information and training to employees in regards to safety data sheets and hazard communication training. Various chemicals are used on the jobsite to maintain equipment, lubricate pipe, and clean the rig. This exposes employees to chemical irritations to eyes, respiratory, and skin due to employee lack of knowledge regarding chemical hazards.
