29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    Employees were exposed to amputation, electrical and stored energy hazards.  There was no written lockout tagout progam when repairing equipment, or performing maintenance, such as, the Pallet Dismantler that takes time to shut down.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  Employees were exposed to struck by hazards in the warehouse as the Toyota electrical forklift was missing light bulbs and lens covers.  The Toyota forklift also had half the seatbelt missing and the back up light was not working.  The forklift had not been taken out of service until it was in safe operating condition.
