On or about July 09, 2019 in the shop area of the facility, employees were exposed to crushing hazards when operating the following press brakes without point of operation guarding: a. Accurpress Hydraulic Press Brake serial #7864 b. Accurpress Hydraulic Press Brake serial #7812 c. Accurpress Hydraulic Press Brake serial #6771 d. Accurpress Hydraulic Press Brake serial #4600 McCorvey Sheet Metal Works, LP was previously cited for a violation of this Occupational Safety and Health Standard 1910.212(a)(3)(ii) which was contained in OSHA inspection number 1321471, citation number 1 item number 5 and was affirmed as a final order on 12/11/2018 with respect to a workplace located at 10221 Rocket Blvd., Orlando, FL, 32824
