29 CFR 1910.333(c)(3): When work was to be performed near overhead lines, the lines were not deenergized and grounded, nor were other protective measures provided before work was started: a) On or about July 4, 2019 and at times prior thereto, prime contractors employees were removing a number of trees along energized power lines and had no protective measures in place. Employee was operating crane in close proximity to power lines. On July 4, 2019 a prime contractor's employee was electrocuted while performing tree removal operations.

29 CFR 1910.333(c)(3)(iii)(A): Vehicles or mechanical equipment capable of having parts of their structures elevated near energized overhead lines were not operated so that a clearance of 10 feet (305 cm) was maintained: a) 118 Pike St. - On or about July 4, 2019 and at time prior thereto, employee was operating a truck mounted crane, within 10' of power lines, lowering tree sections onto the ground for prime contractors employees to trim. On July 4, 2019 a prime contractor's employee was electrocuted while performing tree removal operations.
