29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a) On 07/08/19 -- Worksite located at 6901 NW 80th St, KC MO -- Employees were exposed to electrical shocks and/or burns. Employees were using an extension cord that was missing a ground pin to charge the JLG S-40 aerial lift while they were using it.
