29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.  The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated:    At a jobsite located at 6901 Northwest 80th Street, Kansas City, Missouri 64152, employees were exposed to serious fall hazards. An employee in a genie boom aerial lift was working at elevation without a body belt and lanyard attached to the boom or basket. The employee was exposed to a fall hazard of 14 feet to the ground below.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    At a jobsite located at 6901 Northwest 80th Street, Kansas City, Missouri 64152, employees were exposed to serious fall hazards. Employees were observed working and walking near the edge of a low-sloped roof without the use of any means of fall protection. Employees were exposed to a fall hazard of 23 feet to the ground below.    Hixon Brothers Contracting was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1078171, citation number 1 item number 1 and was affirmed as a final order on 09/04/2015, with respect to a workplace located at 801 Watson Drive, Kearney, Missouri 64060.    Hixon Brothers Contracting was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1060497, citation number 1 item number 1 and was affirmed as a final order on 12/04/2015, with respect to a workplace located at 9301 Collinsville Road, Collinsville, Illinois 62234.

Employees were accessing the roof of a building with a ladder which the side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. Employees were exposed to a fall hazard of 23 feet to the ground below.    Hixon Brothers Contracting was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1060497, citation number 1 item number 1 and was affirmed as a final order on 09/04/2015, with respect to a workplace located at 801 Watson Drive, Kearney, Missouri 64060.
