29 CFR 1910.28(b)(1)(i): Each employee on a walking/working surface having an unprotected side or edge which was four feet (1.2m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Warehouse storage: Employees working near an unprotected edge were exposed to a fall hazard to the concrete floor below. Storage above the freezer: Employees working near an unprotected edge were exposed to a fall hazard to the concrete floor below.

29 CFR 1910.151(c): Where the eyes of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes were not available for immediate emergency use. Warehouse: An emergency eyewash was not available for employees working with corrosive battery acid. Employees were exposed to chemical burns to the eyes.
