29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) 8" Pelletizer: On July 9, 2019 and times prior, the employer used established procedures that were not adequately developed, documented or utilized to address the control of all potentially hazardous energy on equipment such as, but not limited to, the Cumberland 8" pelletizer, which contained gravitational energy and 480V electrical energy. (b) 17" Pelletizer: On or about October 23, 2019 and times prior, the employer used established procedures that were not adequately developed, documented or utilized to address the control of all potentially hazardous energy on equipment such as, but not limited to, the Cumberland 17" pelletizer, which contained approximately 75psi pneumatic energy and 480V electrical energy.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: (a) Line 10: On July 9, 2019 and times prior, employees would perform servicing and maintenance activities on the 8" Cumberland pelletizer without first turning off or shutting down the equipment, to include addressing the gravitational energy as well as the 480V electrical energy. (b) 17" Pelletizer: On or about October 23, 2019 and times prior, employees would perform servicing and maintenance activities on the 17" Cumberland pelletizer without first fully turning off or shutting down the equipment, to include addressing the approximately 75psi pneumatic energy and 480V electrical energy.
