29 CFR 1926.28(a): Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions.  Naval Base San Diego: Gym Renovation Project: An employee was mixing UltraFlex 2 tile mortar containing Portland Cement  with liquid in preparation for floor tile installation wearing blended cloth/neoprene gloves which were not impervious to chemical exposure.  The employee was exposed to dermal caustic burn hazards.
