29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: (a) On or about July 11, 2019, and at times prior, the tongue guard of the 8-inch bench grinder in the maintenance room on the south side of the welding building, was greater than one quarter of an inch from the grinding surface of the abrasive wheel, exposing employees to struck-by and laceration hazards.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: (a) On or about July 7, 2019, and at times prior, employees were exposed to shock hazards from two junction boxes on the back of the control panel for the red compressor that is used to activate the clamps on the rail table, located on the east end of the transition building between the bay doors.
