29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury:    (a)  Flatiron Construction Corp., at 6925 Salvation Lane, Estes Park, CO 80517:  On or before July 9, 2019 the employer did not provide training to employees regarding the hazards associated with the work environment in that employees did not understand and recognize hazards while operating the Roadwidener, Model FH. Employees were exposed to hazards  operating a new piece of equipment attached to the skid steer and standing in the danger zone during operation. Employees were not effectively instructed, nor were procedures implemented for operating the Roadwidener.  This condition exposed employees to struck-by hazards.
