29 CFR 1910.22(d)(1): (1) Walking-working surfaces were not inspected, regularly and as necessary, and were not maintained in a safe condition: a) Ergon Asphalt & Emulsions Plant located at 5445 Hwy 412, Parsons, TN, the dock area along southeastern edge of the facility, along the Tennessee River, on or about 10 Jul 2019: The walking/working platform grate was damaged causing the grate to buckle with a depression of 4 inches with a spread of 22 inches across causing tripping hazard.

29 CFR 1910.25(b)(3): (3) Stairs did not have uniform riser heights and tread depths between landings. a) Ergon Asphalt & Emulsions Plant located at 5445 Hwy 412, Parsons, TN, the dock area along south eastern edge of the facility, along the Tennessee River, on or about 10 Jul 2019: Steps from the Emulsion Plant facility to the loading dock varied in measurements from 7 inches to 14 inches from tread to riser.
