29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: 2nd Floor, north side of building: Subcontractor employees were installing ballistic panels on the walls. An open-sided floor had a top rail that measured 38 inches above the working surface. Employees were exposed to fall hazards of 14 feet to the lower level.

29 CFR 1926.502(b)(2)(i): Midrails were not installed halfway between the top edge of the guardrail system and the walking/ working surface. In the CQC Building, subcontractor employees were installing ballistic panels on the walls. Guardrails were installed along open-sided edges. However, in the following locations, midrails were installed below the required height: a) 2nd Floor, north side of building: The midrail measured 18 inches above the floor where the top rail measured 38 inches. b) 2nd Floor, training shaft: The midrail measured 19.5 inches above the floor where the top rail measured 44 inches. c) 2nd Floor, west side of building: The midrail measured 20 inches above the floor where the top rail measured 44 inches. d) 2nd Floor, east side of building: The midrail measured 20 inches above the floor where the top rail measured 44 inches. Subcontractor employees were exposed to fall hazards of 14 feet to the lower level.

29 CFR 1926.502(b)(6): Guardrail systems were not surfaced as to prevent injury to an employee from punctures or lacerations, and to prevent snagging of the clothing: CQC Building, Second Floor, East Side of Building: Subcontractor employees were installing ballistic panels on the walls. The guardrail installed along the open-sided edge had a split board on the top rail with splinters projecting from the surface. Employees were exposed to lacerations.
