29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   At the jobsite, employees engaged in roofing activities at a height of over 6 feet were not protected by a fall arrest system. Employees were exposed to an unprotected fall of over 22 feet to the ground below.   The Jose Gallegos was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1258665, citation number 1, item number 1 and was affirmed as a final order on 10/06/2017, with a final abatement date of 10/11/2017, with respect to a workplace located at 124 Spruce St. Red Oak, TX 75154.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access;    At the jobsite, employees engaged in roofing activities work were exposed to fall hazards while using a ladder that did not extend at least three feet above the roof's landing.  The Jose Gallegos was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1258665, citation number 1, item number 2 and was affirmed as a final order on 10/06/2017, with a final abatement date of 10/11/2017, with respect to a workplace located at 124 Spruce St. Red Oak, TX 75154.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   At the residential construction site, the employer did not ensure employees installing roofing material were trained to recognize and avoid fall hazards and were exposed to fall hazards of approximately 22 feet.  The Jose Gallegos was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1258665, citation number 2, item number 1 and was affirmed as a final order on 10/06/2017, with a final abatement date of 10/11/2017, with respect to a workplace located at 124 Spruce St. Red Oak, TX 75154.

29 CFR 1926.1060(a): The employer shall provide a training program for each employee using ladders and stairways, as necessary. The program shall enable each employee to recognize hazards related to ladders and stairways, and shall train each employee in the procedures to be followed to minimize these hazards.   At the residential construction site, the employer did not ensure employees installing roofing material were trained to recognize and avoid slip and fall hazards related to ladders.
