Employees engaged in stone installation activities were exposed to a 110 volt electrical shock hazard.  The electrical outlet is missing the knock out, exposing electrical cables.

Employees engaged in stone installation activities were exposed to a 110 volt electrical shock hazard.  The ground pin was missing of an electrical extension use to connect a hand saw.

Employees engaged in stone installation activities were exposed to a 110 volt electrical shock hazard.  Electrical extensions were connected to an outlet without the faceplate exposing employees to live wires.

29 CFR 1926.451(f)(15): Ladders were used on scaffolds to increase the working level height of employees: without satisfying the criteria of subparagraphs (i) - (iv) of this paragraph.  At the residential construction site located at 1321 NE Bradywine, Lee's Summit MO 64064:  An employee was exposed to a fall hazard.  The employee was on top level of the scaffold working from a type A ladder. An employee was working on the ladder exposed to a fall hazard.  The ladder type A was used closed and inclined increasing the probability of fall.
