On or about July 15, 2019, the employer failed to ensure the employee was trained to recognize the hazard and procedures in order to minimize the hazards when operating the JLG aerial lift. The employee was operating and working from the basket of the JLG lift. The employee was not trained on the proper way to operate the JLG and fall protection requirements.

The employee was working below another employees using an air gun and installing boards. The employee was exposed to the hazard of being struck-by the air gun or lumber.

The employee was performing work from the basket and was not wearing fall protection. The employee was exposed to a fall hazard of approximately 11.5 feet to the ground below.
