29 CFR 1926.652(e)(2)(i): Excavation of material to a level no greater than 2 feet (.61m) below the bottom of the members of the support system was allowed when there were indications while the trench was open of a possible loss of soil from behind or below the bottom of the support system: On or about July 15 2019, at an excavation site located on the corner of 5th and Kenosha in Broken Arrow, OK, employees were observed working in an approximately 11 foot deep excavation with the bottom of the protective system elevated more than two feet above the excavation floor. The employees were exposed to struck by and crushed-by hazards.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(c). On or about July 15, 2019, at an excavation site located on the corner of 5th and Kenosha in Broken Arrow, OK, no protective devices were in place to protect employees working in an excavation that was more than 5 feet in depth, exposing employees to struck-by and crushed-by hazards. R.L. Hensley Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 987189, citation number 1, item number 2, and was affirmed as a final order on October 9, 2014, with a final abatement date of October 17, 2014, with respect to a workplace hazard located at 7300 E 121st Place South in Bixby, OK 74008 R.L. Hensley Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1152105, citation number 1, item number 3, and was affirmed as a final order on September 16, 2016, with a final abatement date of August 25, 2016, with respect to a workplace hazard located at Highway 169 North South of 56th Street Exit in Tulsa, OK 74114
