29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   (a)  Jobsite:  On or about July 15, 2019, an employee was not wearing a hard hat while he was working from the ground and other employees were using hand tools above him on the roof.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Jobsite:  On or about July 15, 2019, employees were working on the roof of several adjoining homes and they were not protected from falling.  The pitch of the roof was about 6 in 12 and the fall hazard to employees ranged from about 10 feet to about 26 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:  (a)  Jobsite:  On or about July 15, 2019, employees need to be retrained so that they retain the knowledge and skills required on when and how to use fall protection when working on a roof.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a)  Jobsite:  On or about July 15, 2019, employee were accessing/egressing the roof by using ladders that did not extend at least 3 feet above the edge of the roof.  Two ladders extended about 6 inches above the edge of the roof and one ladder extended about 2 feet above the edge of the roof.  The fall hazards ranged from about 10 feet to about 20 feet.

29 CFR 1926.1060(b): The employer did not provide retraining as necessary for each employee using ladders and stairways so that the employee maintained the understanding and knowledge acquired through prior training:  (a)  Jobsite:  On or about July 15, 2019, the employer needs to retrain its employees in the use of ladders.  Employees were using ladders that were not set up properly to access the roof.
