29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end: a) 1704 State Street, Administrative Building, entire roof, on or about 7-15-19: Employees were working on multiple roof jack scaffold platforms with planks that were extended more than 18 inches beyond their supports, exposing the employees to fall hazards of 13 to 18 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) 1704 State Street, Administrative Building, entire roof, on or about 7-15-19: Employees were on a sloped roof (22:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 12 feet 10 inches from the eave to the ground and/or 26 feet from the gable end peak to the ground.
