29 CFR 1926.502(d)(15)(ii):Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person. a) Splash Carwash, 26424 Johnson Road, Evans Mills, NY 13637: On the roof. On or about 3/16/23, a worker was installing standing seam metal roof components and slipped . The worker used a personal fall arrest system (harness, lanyard, lifeline with rope-grab, and anchor) which failed at the connection of the anchorage. The worker fell 13.5 feet to ground and struck the railing of a scissor lift.
