Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse. a) Warehouse (Row 50) - On or about July 16, 2019, and at times prior, employees are exposed to struck-by and/or crushed hazards, in that multi-level palletized storage rack was bent/damaged while employees working around the racks and operating the forklift and stand-up lift trucks.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) Warehouse - On or about July 16, 2019, and at times prior, the employer exposed employees to struck-by hazards in that, powered industrial truck operators were allowed to operate Crown stand-up lift trucks and Landoll Drexel forklifts for loading and unloading the boxes on pallets without having received formal instruction, training and operator evaluation to determine the retention of the information.
