29 CFR 1910.22(a)(2): The employer did not ensure the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. When wet processes were used, drainage was not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats were not provided: a) Production Area, Entryway: On July 15, 2019, the employer exposed employees to falls to the same elevation in that drainage was not maintained as employees walked over sanitizer foam in order to disinfect their shoes upon entering and exiting the production area. b) Production Area, Conveyor Lines: On July 15, 2019, the employer exposed employees to falls in that walking-working surfaces were not kept clear of water and loose lettuce that had accumulated as a result of processing romaine.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: (a) Processing Area, Line 3: On July 15, 2019, and at times prior, the employer exposed employees to caught in hazards in that a rotating shaft on the line 3 conveyor belt system was unguarded as employees worked at the machine cutting produce.
