29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Polish Department, on or about 7/17/19: Polishers, such as but not limited to, Bader #1, Bader #5 and Bader #9, had guards that were not properly adjusted to protect the operator from the hazards created by the ingoing nip points of the belts and rollers. b) CNC Department, on or about 7/17/19: Okkuma CNC Machine #221 and #222 are being operated with the door safety interlocks disconnected, thereby allowing operators access to inside the machine during operation.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch: a) North Production Area, on or about 7/17/19: Baldor grinder was equipped with an adjustable tongue guard on the right side that was positioned 5/8-inches from the abrasive wheel. b) Tool Room, on or about 7/17/19: Baldor grinder was not equipped with an adjustable tongue guard on the left side. The distance between the abrasive wheel and the safety guard peripheral member at the top was 1-3/8 inches.

29 CFR 1910.219(c)(2)(i): All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires. a) Tool Room, on or about 7/17/19: Baldor buffing machine was not equipped with guards that covered the rotating shafts, spindle ends and nuts. b) Polish Department, on or about 7/17/19: Six (6) Baldor buffing machines were not equipped with guards that covered the rotating shafts, spindle ends and nuts.
