29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee use appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors or potentially injurious light radiation.    On or about July 15, 2019, employees engaged in residential roofing operations were exposed to struck by hazards while operating pneumatic nail guns without eye protection.      No abatement certification or documentation is required for this item.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about July 15, 2019, employees engaged in residential roofing operations up to approximately 13 feet above grade were exposed to fall hazards.  Employees had not received adequate training to recognize and minimize fall hazards on the jobsite.     Abatement certification required for this item in accordance with 29 CFR 1903(c).

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about July 15, 2019, employees were exposed to fall hazards while using an extension ladder to access/egress the roof that did not extend three feet above the roof eave/landing surface.   Abatement certification is not required for this item.
