29 CFR 1926.252(a): An enclosed chute of wood, or equivalent material, was not used where materials were dropped more than 20 feet to points lying outside the exterior walls of the building(s):  a) On September 3, 2020, waste roofing materials were dropped approximately 26 feet from the roof of the building into roll-off containers below without an enclosed chute.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a)  On September 3, 2020, employees engaged in roofing activity were not protected from fall hazards while working at an elevation of approximately 26 feet above the ground.  Tambo Construction Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1416091, citation number 1, item number 2 and was affirmed as a final order on September 18, 2019, with respect to a workplace located at 123 E. Hawthorne Blvd., Wheaton, Illinois 60187.  No abatement certification or documentation is required for this item.
