29 CFR 1910.134(d)(1)(i): Selection of appropriate respirators was not based on the respiratory hazard(s) to which the worker was exposed and user factors that affect respirator performance and reliability: (Construction Reference 1926.103) (a) Euro Paint LLC, on or about June 13th, 2019, at the location of 13381 Ludwig Road, Grand Rapids, OH, 43522: Employees tasked with bridge blasting were not provided with a respirator appropriately selected based on their exposure to a respiratory hazard of 59,600 micrograms of lead per cubic meter for an 8-hour time weighted average. The employees were provided with Nova 2000 blast helmets which have a maximum use concentration of just 50,000 micrograms of lead per cubic meter, potentially over-exposing employees to lead.

Employees wore a 3M 7500 series respirator underneath of a Nova 2000 Blast Helmet. There is the potential that the use of these two respirators together could affect the protection factors of each other due to their mutual interference, potentially causing employees to be unprotected from the lead overexposure.

The employer did not ensure that employees were trained to recognize how this improper usage could compromise the protective effect of the respirator. Employees wore a 3M 7500 series respirator underneath of a Nova 2000 Blast Helmet. There is the potential that the use of these two respirators together could affect the protection factors of each other due to their mutual interference, potentially causing employees to be unprotected from the lead overexposure.

29 CFR 1926.62(i)(2)(i): The employer did not provide clean change areas for employees whose airborne exposure to lead was above the permissible exposure limit, without regard to the use of respirators: (a) Euro Paint LLC, on or about July 18th, 2019, at the location of 13381 Ludwig Road, Grand Rapids, OH, 43522: Employees tasked with bridge blasting were evaluated by the employer as being overexposed to the permissible exposure limit for lead and not provided with clean change areas. Lead was found at 32.41 micrograms per 100 square centimeters on the clean side of the shower trailer used for changing, which was in excess of the HUD's acceptable decontamination level of 200 ug/ft2 (21.5 micrograms per 100 square centimeter).

29 CFR 1926.62(i)(4)(ii): The employer did not ensure that lunchroom facilities or eating areas were as free as practicable from lead contamination: (a) Euro Paint LLC, on or about July 18th, 2019, at the location of 13381 Ludwig Road, Grand Rapids, OH, 43522: Employees tasked with bridge blasting, painting, and related activities were permitted to eat lunch in a designated area at the worksite. Lead was found at 97.1 micrograms per 100 square centimeters on the handle of a cooler in this area, which was in excess of the HUD's acceptable decontamination level of 200 ug/ft2 (21.5 micrograms per 100 square centimeter).
