29 CFR 1926.62(h)(1): All surfaces shall be maintained as free as practicable of accumulations of lead: On or about May 23, 2023, at a support area, located at 1260 Hurlbut Street in Green Bay, WI, for a worksite located on the Leo Frigo Memorial Bridge on Interstate 43 in Green Bay, WI; the employer did not maintain all surfaces of a decontamination trailer as free as practicable of accumulations of lead. The floor and bottom of a locker on the clean side of the decontamination trailer contained accumulations of lead of approximately 76,223 �g/ft^3 and 1094 �g/ft^2 respectively.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about June 15, 2023, at a worksite located on the Leo Frigo Memorial Bridge on Interstate 43 in Green Bay, WI; the employer did not protect employees from falling from the unprotected edge of the bridge deck to the suspended temporary work platform approximately 11.5 feet below. Employees working between the compressor equipment or the abrasive blasting equipment and concrete K-rails (a.k.a. Jersey Barrier) at the deck's edge were not protected by a guardrail system, safety net system or personal fall arrest system.

29 CFR 1926.1051(a): A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided. On or about June 9, 2023, at a worksite located on the Leo Frigo Memorial Bridge on Interstate 43 in Green Bay, WI; the employer did not provide a stairway or ladder at the personnel point of access for the suspended temporary platform below the bridge deck. Employees had to step across, approximately 20 inches, and down, approximately 28 inches, from the K-rails (a.k.a. Jersey Barriers) to a bridge girder before being able to access a ladder to the suspended temporary platform.
