1926.21(b)(2) The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about July 18, 2019, near 10055 N. I-35 Service Road in Oklahoma City, OK, an employee was allowed to work in an excavation without adequate protective systems in place, exposing the employee to struck-by and crushed-by hazards.
