29 CFR 1910.176(b): Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse: a) Warehouse: On or about July 17, 2019 and at times prior, employees were exposed to struck by hazards while working in and around storage racks that were not securely fastened to the floor. b) Warehouse: On or about July 17, 2019 and at times prior, employees were exposed to struck by hazards while working in and around storage racks that did not display a plaque designating load sizes. c) Shop: On or about July 17, 2019 and at times prior, employees were exposed to struck by hazards while working in and around storage racks that were not securely fastened to the floor. d) Shop: On or about July 17, 2019 and at times prior, employees were exposed to struck by hazards while working in and around storage racks that did not display a plaque designating load sizes.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: (a) (Shop) On or about July 17, 2019 and at times prior, employees were exposed to struck by hazards while performing grinding operations on the abrasive wheel side of a Dayton Bench Grinder (Model #1Z707, Serial # 60064-B 1W) whose work rest was greater than 1/8" away from the wheel.
