29 CFR 1926.20(b)(1): The employer did not initiate and maintain accident prevention programs as necessary to comply with this part:  a) Roberto Hernandez located at 5269 Guilford Lane Delaware, OH 43015: On and preceding 16 July, 2019, employees were framing the exterior sheathing and other various framing pieces on the roof of a two story residential home. The employer did not initiate and maintain safety and health programs related to the framing work being conducted by employees.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury.  a) Roberto Hernandez located at 5269 Guilford Lane Delaware, OH 43015: On and preceding 16 July, 2019, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the procedures to be followed in order to minimize these hazards.  Framers were conducting various tasks at heights up to 20 feet using pneumatic nail guns.  The employees were not utilizing proper eye or fall protection which exposed the them to falls up to 20 feet and struck by hazards to the eye and face.

The following condition exposed employees struck by hazards to the eyes.

Framers were observed installing sheathing and other framing pieces on the main roof and in the sections between trusses without fall protection.  This condition exposed employees to falls up to 20 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Roberto Hernandez located at 5269 Guilford Lane Delaware, OH 43015: On and preceding 16 July, 2019, the employer did not ensure that employees were provided with a ladder adequate in height to extend 3 feet above the upper landing.   Employees working on the roof of 2 two story residential home up to 20 feet above the lower level were utilizing an 8 foot A-frame ladder for access that was 2-3 feet below the upper landing causing them to use the top step of the ladder to get high enough to gain access. This condition exposed employees to falls up to 8 feet.

Employees working on the roof of 2 two story residential home up to 20 feet above the lower level were utilizing and A-frame ladder for access that was 2-3 feet below the upper landing causing them to use the top step of the ladder to get high enough to gain access. This condition exposed employees to falls up to 20 feet.
