On or about July 18, 2019, on the 6:12 pitch roof, 4 roofing employees installing roofing shingles were exposed to a 10.5 feet fall hazard from the eave and a 13 feet fall hazard from the raked edge of the front gable in that they were not protected by a fall protection system.
