29 CFR 1910.147(c)(7)(iv): The employer's certification of employee training did not contain each employee's name and date of training:    a) 111 Chimney Rock Road - Production    The employer did not certify that authorized and affected employees were trained on lock-out/tag-out procedures. Employees performed maintenance to equipment and machines such as but not limited to the milling machines with multiple energy sources (electrical and pneumatic).    Violation noted on or about 7/24/19    WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   a) 111 Chimney Rock Road, Bridgewater, NJ 08706 - Building 3  The employer did not provide information on Appendix D of 29 CFR 1910.134 to employees who voluntarily used N95 dust masks while performing work duties such as but not limited to line marking and color striping pressure tubes.  Condition observed on or about 7/24/19.
