The employer is failing to prevent employee exposure to struck-by / crushing hazards. At 6190 Mid Rivers Mall Drive in St. Peters, MO 63304 - On or about 7/23/19, employees were exposed to struck-by / crushing hazards where operators were allowed to travel with the mast of a "Crown" (Model No. RR5725-45; Serial No. 1A422917) powered industrial truck (PIT) raised approximately 16 feet while loaded with about 26 sheets of 5/8-inch drywall, measuring 48 inches in width by 144 inches in length, and a combined weight of approximately 2,745.6 pounds, to clear the height of a shelving system and other stacks of sheet materials and sales products, which reduced the aisle opening (width) to approximately 138 inches.
