OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees.    a). Machine Shop (adjacent to the OKKA, OKKB, Tongtai HB-500 [Serial No. 027869], and Tongtai HB-630 machines) - On or about and times prior to November 8, 2019, the hook latches on four Dayton hoist systems were damaged, thus exposing employees to struck-by hazards.     b). Machine Shop (adjacent to the OKUMA A machine) - On or about and times prior to November 8, 2019, the hook latch was missing on a Dayton hoist system, thus exposing employees to stuck-by hazards.    c). Machine Shop - On or about and times prior to November 8, 2019, pre-operational and frequent inspections were not conducted on any of the Dayton and CM Lodestar hoist systems, thus exposing employees to struck-by hazards.

29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by the use of guardrail systems, safety net systems or personal fall arrest systems.    a). De-Gate Area - On or about and times prior to September 10, 2019, the employer did not provide a fall protection system to employees that worked from an elevated platform that had an unprotected edge, thus exposing employees to a fall hazard of approximately 7 feet 3 inches.

Molding Department, 2nd Level - On or about and times prior to September 10, 2019, the employer did not ensure employees were protected from tripping or stepping into a floor hole, thus exposing employees to a trip and a fall hazard of approximately 23.5 inches.

At the workplace - On or about and times prior to July 13, 2019, the employer failed to ensure adequate procedures were developed for forklift maintenance activities that were conducted with the forklift carriage assembly elevated, thus exposing employees to a struck-by hazard.    b). At the workplace - On or about and times prior to July 13, 2019, the employer failed to ensure adequate training was provided to maintenance personnel performing routine forklift maintenance activities, thus exposing employees to a struck-by hazard.    Among other methods, one feasible means of abatement would be to send employees to manufacture certification course.
