29 CFR 1904.7(a): 1904.7(a) The employer did not record workplace injuries and illnesses that would be considered to meet the general recording criteria, and therefore to be recordable, if it results in any of the following: death, days away from work, restricted work or transfer to another job, medical treatment beyond first aid, or loss of consciousness. Entire Facility. On or about July 6, 2019 through July 23, 2019, the employer did not record on the 2019 OSHA 300 Log the following work-related illness cases that met the general recording criteria when employees incurred heat exposure: a) On 7/6/2019 at approximately 1:25 p.m., an employee lost consciousness in the workplace and received emergency medical treatment. b) On 7/8/2019 at approximately 8:46 p.m., an employee felt faint and hot in the workplace and received emergency medical treatment. c) On 7/16/2019 at approximately 9:20 p.m., an employee lost consciousness in the workplace and received emergency medical treatment. There is no abatement certification or documentation required for this item.

29 CFR 1904.7(b)(4): When an injury or illness involved restricted work or job transfer but did not involve death or days away from work, the employer did not correctly record the injury or illness on the OSHA 300 Log by placing a check mark in the space for job transfer or restriction and/or an entry of the number of restricted or transferred days in the restricted workdays column. Entire Facility: On or about July 10, 2019 through July 23, 2019, the employer did not correctly record check marks and number of restricted days for each of the following cases on the 2019 OSHA 300 Log that resulted in restricted work activity: a) On 7/10/2019 an employee was injured when struck by a forklift while sorting freight. There was no check mark in Column I and the number of restricted days was not shown in Column L. b) On 7/15/2019 an employee was injured when pulling a pallet jack when another employee pushed a pallet into her right ankle. There was no check mark in Column I and the number of restricted days was not shown in Column L. c) On 7/20/2019 an employee pinched her finger when replacing a propane tank on a forklift. There was no check mark in Column I and the number of restricted days was not shown in Column L. There is no abatement certification or documentation required for this item.
